Limitations of C-urea Breath Test (ubt) and Stool Antigen Test (sat) in Diagnosis of Helicobacter Pylori (hp) Infections

چکیده

Helicobacter pylori (HP)(1) management (including diagnosis and therapy) has been exhaustively reviewed in several reports. These reports are unanimous in that several clinical conditions seriously hamper the diagnostic value of two HP tests: 13 C-Urea Breath Test (UBT) and Stool Antigen test (SAT), because of the false-negative results frequently (up to 40%) obtained in these conditions. Basically, these false-negative results are due to decreased bacterial loads in the stomach mucosa, and include the following clinical conditions: 1) use of PPI medication; 2) use of antibiotics; 3) bleeding peptic ulcer; 4) atrophic gastritis (with or without intestinal metaplasia); 5) gastric cancer; 6) MALT lymphoma, and 7) partial gastrectomy. Since the late 1990’s, it has been well established that UBT also gives falsepositive results in cases where urease-producing bacterial species are colonizing an achlorhydric stomach due to atrophic gastritis or a long term use of proton pump inhibitors (PPIs).

Evaluation of [13C]urea breath test and Helicobacter pylori stool antigen test for diagnosis of H. pylori infection in children from a developing country.

The [(13)C]urea breath test ((13)C-UBT) and Helicobacter pylori stool antigen test (HpSA) for the diagnosis of H. pylori infection in children were validated. The sensitivity, specificity, and positive and negative predictive values were 93.8, 99.1, 97.8, and 98.0%, respectively, for the (13)C-UBT and 96.9, 100, 100, and 98.0%, respectively, for HpSA. Both tests are appropriate for diagnosing H...

Stool antigen detection versus 13C-urea breath test for non-invasive diagnosis of pediatric Helicobacter pylori infection in a limited resource setting

Introduction The prevalence of childhood infection with Helicobacter pylori is high, especially in developing countries. Non-invasive methods for detection of infection in children should be inexpensive, easy to perform, well tolerated and have a high diagnostic accuracy. We aimed to compare the reliability, specificity and sensitivity of the H. pylori stool antigen (HpSA) test with the 13C-ure...

Diagnostic Methods of Helicobacter pylori Infection for Epidemiological Studies: Critical Importance of Indirect Test Validation

Among the methods developed to detect H. pylori infection, determining the gold standard remains debatable, especially for epidemiological studies. Due to the decreasing sensitivity of direct diagnostic tests (histopathology and/or immunohistochemistry [IHC], rapid urease test [RUT], and culture), several indirect tests, including antibody-based tests (serology and urine test), urea breath test...

Comparison of non-invasive tests to detect Helicobacter pylori infection in children and adolescents: results of a multicenter European study.

OBJECTIVE To compare the current non-invasive tests for Helicobacter pylori infection in children and adolescents. STUDY DESIGN This multicenter, multinational study investigated the sensitivity, specificity, and positive and negative predictive values of four non-invasive tests: urea breath test (UBT), stool antigen test, and antibody detection in serum and urine, in comparison with biopsy-b...

Conflicting results of non-invasive methods for detection of Helicobacter pylori infection in children with celiac disease - a preliminary study.

BACKGROUND There are no data addressing the usefulness of non-invasive tests for the detection of Helicobacter pylori (HP) infection in celiac disease (CD). AIM The aim of this study was to compare two most sensitive and specific tests - urea breath test (UBT) and fecal antigen test (FAT) in HP diagnosis in CD patients. MATERIALS AND METHODS The study comprised of 76 CD patients, 49 healthy...
